This review discusses detector physics and Monte Carlo techniques for
cryogenic, radiation detectors that utilize combined phonon and ionization
readout. Particular focus is placed on instrumentation that is used in the
Cryogenic Dark Matter Search detectors however the discussion is quite general
and includes phonon and charge transport physics relevant at low temperatures.
Sufficient tutorials and physics references are provided such that an
interested reader can jump right into a detector Monte Carlo campaign.
